Paid Vacation Request Form

As a Lloyd Staffing associate, you can earn a bonus check after working 1,250 hours within a 12-month period. Your bonus check is based on your average hourly wage and average weekly hours up to a maximum of 35 hours (not including overtime) for the 12-month period you have worked. Only one vacation check is issued per one-year period. Requests for vacation pay must be made either in writing or via the following form. (Please note there is a cap on the vacation check issued based on skill category.)
